The directory includes a list of our physicians, office locations, and the health plan code you’ll need when selecting a physician.
2. Enroll Using Your Health Plan
Use the health plan code from the directory on your enrollment form or when signing up over the phone.
Already enrolled? You can switch to a SoCal Care IPA doctor by contacting your health plan and requesting to join.
